COMPLAINT PROCEDURE

UNIFORM COMPLAINT PROCEDURE

Any individual, public agency or organization may file a written complaint of alleged noncompliance with the District by completing the appropriate notification form. The Uniform Complaint Form may be obtained from Amanda Belcher, Executive Assistant to the Superintendent, at the District office at 414 E. Ojai Avenue, Ojai, CA 93023 or by clicking on links below.

UNIFORM COMPLAINT POLICY

The District designates the individual identified below as the employee responsible for coordinating the District's response to complaints and for complying with state and federal civil rights laws. The individual also serves as the compliance officer. The individual shall receive and coordinate the investigation of complaints and shall ensure district compliance with the law.

The compliance officer who receives a complaint may assign another compliance officer to investigate the complaint. The compliance officer shall promptly notify the complainant if another compliance officer is designated to investigate the complaint.

CRISIS MANAGEMENT:

Upon notification of a crisis situation related to students or staff, site administration will refer to the appropriate emergency protocols listed in the Appendix of the OUSD Student Wellness Policy. In the event information needs to be shared, every effort will be made to consider the family’s wishes as related to disseminating information to the student body or staff. Confidentiality by all staff will be maintained, and no information or action will occur without prior approval from the school’s administration. It is a priority for our school district to consider privacy and sensitivity relative to the circumstance.
