There are two important steps to take in the case of a positive COVID case or exposure to a close contact with COVID.
If the chart below indicates your student should stay homes, please fill out this absence form .
In order to return to school after a COVID exposure or having COVID, follow the guidance in the chart below. In most cases, you will need to complete this Return to School form.

All Ojai Unified employees and students can COVID Test at any District Site or use any at-home test kit to complete the Return to School Form
Optional on-site COVID testing for students is available at all sites. Students and staff will have the opportunity once per week to take a COVID test using either a cheek or nostril swab. This service is free to all staff and students* and is not mandatory.

If you would like your student to participate in our optional, district-wide and free COVID testing, please follow these steps (updated 1/12/2022):
Information for Community Members
1. School site testing is for students and staff, designed to keep in-person learning a priority. We may be able to provide testing to community members, only after all staff and students have been processed. Again, this provision is based on capacity and time permitting.
2. Please remain in the designated testing area. If you are other places on campus outside of the designated testing area you will be asked to leave.
3. You will need to bring copies of the appropriate forms, including your insurance card. We will not be able to make copies for you.
4. Given our commitment to use our resources for our staff and students, when these guidelines are not followed, sites may be closed to future drop-in testing.
Important Testing Forms
Complete Patient Test Request form in English and Spanish here . We suggest that you fill out the form, sign it, and leave the date blank.as you will need to provide this form each time you test, along with either:
a copy of proof of insurance, or
a copy of this uninsured patient form in English and Spanish here
Students under the age of 18, must also submit this Parent Permission Form (only once) here in English and here in Spanish.
Health insurance is not required for testing. However, if you do not have health insurance, the above form is necessary so that PMH can utilize CARES act funding. If you have insurance, testing will have no effect on your personal policy, your deductible, or generate a co-pay.

*If you have Blue Shield Blue Cross insurance and were improperly for COVID testing through Ojai Unified, please let us know by emailing covidconcerns@ojaiusd.org.and we can help direct you to the right person to fix the issue.
